Name of Product: Discovery Kids(tm) Animated Marine and Safari Lamps

Units: About 300,000

Importer: Innovage LLC, of Foothill Ranch, Calif.

Hazard: The placement of internal wires near the circuit board can cause electrical short-circuiting and sparking, posing a fire and a burn hazard to consumers.

Incidents/Injuries: Innovage has received 11 reports of short circuiting. This includes three reports of lamps catching fire, which led to property damage. No injuries have been reported.

Description: This recall involves Discovery Kids Animated Marine and Safari Lamps that feature rotating films with marine or safari scenes. The words “Discovery Kids” are printed on the front top-left corner of the product. The recalled products have both an 11-digit batch number that begins with either 584894 or 10128 and a model number of 1628626, 1642433, 1641522, 1641523, 1645729, or 1645853. Batch numbers can be found imprinted in the plastic underneath the lamps and on the bottom of the packaging. Model numbers can be found on stickers placed underneath the lamps and on the bottom of the packaging near the barcode.

Sold at: Bed Bath & Beyond, Bonton, JCPenney, Kohls, Office Max, Toys “R” Us stores nationwide as well as the online retailers Amazon, Ideeli, JCPenney, Kohls, Macy’s and Overstock from June 2010 through March 2012 for between $10 and $20.

Name of Product: Twist’n Sparkle Home Beverage Carbonation System plastic bottles.

Units: About 162,700

Importer: iSi North America Inc, of Fairfield N.J.

Hazard: The plastic bottles can explode under pressure, expelling plastic parts, resulting in an injury hazard to anyone nearby.

Incidents/Injuries: The firm is aware of nine incidents involving exploding plastic bottles including three in which consumers received cuts to various parts of their upper body.

Description: The products are plastic bottles used as a part of the iSi Twist’n Sparkle Beverage Carbonation System. The recalled bottles were sold in the Starter Set model number 1005 with one reusable bottle and the Bottle Set model number 1006 with two reusable bottles. The model numbers are printed on the bottom of the box. The recalled plastic bottles are available in one size and two colors of caps/bottoms, white or gray.

Sold at: Williams-Sonoma, QVC and other national retailers and websites from June 2010 to March 2012 for approximately $50 for the Starter Set and $30 for the Bottle Set.

Manufactured in: Austria and Hungary

Remedy: Consumers should immediately stop using the recalled products and either contact iSi or the place of purchase for instructions on returning the product for a refund or store credit. For products purchased online, contact the online retailers for instructions on how to ship the returns and receive a refund or credit. Note that the US Postal Service does not accept CO2 gas chargers for shipment by mail.

Manufacturer: Bath Petals Inc., of Gardena, Calif.

Hazard: The candle can burn with a high flame, causing excessive heat. This poses a fire hazard. The heat and flame can cause the glass candle holder to shatter. This poses a laceration hazard.

Incidents/Injuries: The firm has received one report of a candle burning with a high flame and shattering the glass holder. No injuries or property damage were reported.

Description: The candles are 7.5 oz. soy candles sold in four colors and scents: Australian Eucalyptus, California Rose Garden, French Alpine and Thai Lemongrass Ginger. “Bath petals” and the scent name are printed on the glass candle holder. The following UPC codes are on the bottom of the box: 6-10696-55269-3, 7-97734-03754-8, 7-97734-03755-5 and 7-97734-03758-6

Sold at: TJ Maxx and Marshalls in the U.S. from February 2012 through April 2012 and at Homesense, Winners and Marshalls in Canada in April 2012 for about $10.

The SkilSaw logo appears at the top of the upper blade guard and on the dust collection bag.

Name of Product: Nikon digital SLR camera battery packs

Units: About 5,100 in the United States, 1,100 in Canada and an additional 195,000 worldwide

Distributor: Nikon Inc., of Melville, N.Y.

Hazard: The battery packs can short circuit, causing them to overheat and melt, posing a burn hazard to consumers.

Incidents/Injuries: Nikon has received seven reports of incidents outside of the U.S. and Canada of the recalled battery packs overheating. No incidents have been reported in the U.S. or Canada. No injuries have been reported.

Description: This recall involves Nikon EN-EL 15 rechargeable lithium-ion battery packs with lot numbers E and F. The battery pack was sold with the Nikon digital SLR D800 and D7000 model cameras. The battery pack’s model number “EN-EL15″ and “7.0V 1900mAh 14Wh” are printed on the back of the battery pack. Only battery packs with an “E” or “F” in ninth character of the 14-digit lot number located on the back of the battery pack are included in this recall.

Sold at: Camera, office supply and mass merchandise stores, in catalogs and on various websites nationwide. They were sold with the digital SLR camera in Canada from February 2012 through March 2012 and in the U.S. from March 2012 through April 2012 for between $1,200 and $3,000.

Manufactured in: Japan and China

Remedy: Consumers should stop using the recalled battery packs immediately, remove them from the camera and contact Nikon for a free replacement battery pack.

Stage and Riser Caddies Recalled by Midwest Folding Products Due to Risk of Injury or Death

A 3-year-old girl died in May 2011 in Greenville, S.C., from severe head trauma when a portion of a portable stage fell out of the caddy and onto her at a church.

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Midwest Folding Products, of Chicago, Ill., is announcing a voluntary recall of about 5,300 stage and riser caddies due to risk of injury and death to consumers. The caddy’s latches do not automatically close to secure the stages and risers when stored. Unsecured stages and risers can fall off the caddies and onto consumers.

A 3-year-old girl died in May 2011 in Greenville, S.C., from severe head trauma when a portion of a portable stage fell out of the caddy and onto her at a church.

This recall involves Midwest Folding Products’ caddies used to store collapsible stages and risers when they are not being used. The caddies are brown, gray or black metal and measure 44 inches long by 30 inches wide by 60 inches high. They hold six 96-inch long or eight 48 to 72-inch long stage or riser platforms.

Yellow caution and instruction labels are located on the caddy’s frame.

The recalled caddies were sold by Midwest Folding Products’ dealers to churches, schools, performing arts centers and various other organizations nationwide and online at www.midwestfolding.com from January 1989 through March 2012 for about $1,200. Midwest Folding Products is directly contacting known purchasers of the caddies.

Caddy owners and consumers should immediately stop using the recalled stage and riser caddies. Caddy owners should contact Midwest Folding Products to receive a free repair kit with new brackets that will allow latches to automatically return to the closed position on the caddies and new white warning and instruction labels.

Description: This recall involves Heath®/Zenith 270° Motion Security Lights with model number SL-5414-WH. It replaces a standard outdoor wall mounted light fixture and is designed to turn on when motion is detected at night. The product has two cone-shaped lightbulb receptacles attached to a round mounting plate. A small, rectangular motion sensor is attached to the mounting plate between the lightbulb receptacles. The unit is made of aluminum and plastic. It comes in white only. The brand name and model number are on a label located on the back of the motion sensor.

Sold Exclusively at: Home Depot Stores from September 2011 through April 2012 for about $30.

Units: About 50,000 in the United States and 2,500 in Canada

Hazard: The wheels and hubcaps on the toy cars can detach, posing a choking hazard to young children.

Incidents/Injuries: Green Toys has received 10 reports of wheels and hubcaps detaching or loosening from the vehicles. There have been no reports of injuries.

Description: This recall involves Green Toys(tm) Mini Vehicles. All cars are made of plastic and available in yellow, green, red, white and blue. Model names and numbers for the seven recalled vehicles or sets are listed in the table below. For Mini Vehicle (MVHA – 1014) and Fastback (MVFA-1022) sets the model numbers can be found on the bottom of the toys’ packaging. For all other cars, a label with the model number can be found on the bottom of the car itself.

Cars with an “I” etched into the underside of the car next to the date stamp are not part of the recall.

Sold by: Mass merchandisers, grocery stores and independent toy and specialty stores nationwide and by online retailers from April 2012 to June 2012. The cars cost about $5 each or $18 for the set.

Name of Product: Kenta and Kenta Plus child carriers

Units: About 300 in the United States and 100 in Canada

Hazard: The side strap’s seam can unravel and cause the strap to separate, posing a fall hazard to the child in the carrier.

Incidents/Injuries: VAUDE has received two reports of the side strap separating. No injuries have been reported.

Description: This recall involves backpack-style child carriers sold under the Kenta and Kenta Plus model name. “VAUDE” is printed on the back and between the shoulder straps of the carrier. “Kenta” or “Kenta Plus” is printed on the lower back or hip belt of the carrier. The carriers were sold in black, red and brown and were designed for children up to 31 pounds.

Sold at: Outdoor camping and climbing stores nationwide from January 2010 through May 2012 for about $150 for the Kenta and $170 for the Kenta Plus models.
