WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Bugaboo Cameleon and Bugaboo Donkey Model Strollers

Units: About 46,300 in the U.S. and 4,440 in Canada

Importer: Bugaboo Americas, of El Segundo, Calif.

Manufacturer: Bugaboo International B.V., of Amsterdam, The Netherlands

Hazard: A button on the stroller’s carrycot/seat carry handle can become disengaged and cause the handle to detach, posing fall and choking hazards to young children.

Incidents/Injuries: Bugaboo has received 58 reports of carry handles detaching. No injuries have been reported.

Description: This recall involves the carry handles on Bugaboo Cameleon and Bugaboo Donkey model strollers with detachable carrycots/seats. The strollers were sold with a base, sun canopy and other accessories in various colors. A fabric tag on the side of the sun canopy has Bugaboo and the model name. Strollers included in the recall have a serial number that falls within the range listed below. Serial numbers are printed on the stroller’s chassis, located under the carrycot/seat.

The Bugaboo Cameleon was sold between September 2009 and June 2012 for about $980. The Bugaboo Donkey was sold from January 2011 through December 2012 for between $1,200 and $1,600.

Remedy: Consumers should immediately remove the carry handle from the strollers and contact Bugaboo for a free replacement handle. Consumers can continue to use the strollers while awaiting the replacement handle.

Description: This recall involves Class Club brand children’s letterman jackets. The 100% polyester jackets were sold in navy with white sleeves in sizes 2 to 3. A patch on the upper, right chest of the jacket reads “28 F Football Athlete State Champs.” Style number “F14CL112L” is printed on the back of the sewn-in size label in the back neck.

Description: This recall involves “Pink Angel” branded embroidered denim shorts for girl infants and toddlers in sizes 12M to 4T. The shorts come with a bright pink lace belt and feature two embroidered butterflies on the left front panel with decorative studs attached to the front and rear pockets. Style numbers B91540 or B11540 can be found on the hangtag of the shorts.

Name of Product: Children’s Water Bottle

Units: About 2,900

Importer: H&M Hennes & Mauritz, New York, N.Y.

Hazard: The water bottle’s spout can break off, posing a choking hazard to children.

Incidents/Injuries: H&M has received one report of an incident in England of the water bottle spout breaking off in a child’s mouth as the child was drinking from the bottle. No injuries have been reported.

Description: The 16 oz. water bottles are pink plastic with a crackle design or blue plastic. The bottles have flip-top lids in coordinated pink and blue colors. “H&M Sweden” and “www.hm.com” are embossed on the bottom of the bottle. The recalled products were manufactured in June 2012. The water bottle contains the manufacture date in an embossed date clock on the bottom of the bottle. The inner circle on the clock contains the number 12 with an arrow between the two numbers. The arrow points at the number 6 in an outer circle of numbers.

Sold exclusively at: H&M stores with children’s departments nationwide from July 2012 through August 2012 for about $5.

Manufactured in: Italy

Remedy: Consumers should immediately stop using the water bottle and return it to H&M for a full refund.

Incidents/Injuries: Toys “R” Us has received eight reports of the knobs detaching. No injuries have been reported to the firm.

Description: This recall involves the Imaginarium 5-Sided Activity Center. The multi-sided center has two triangle-shaped ends with a wooden multi-colored xylophone on one and a mirror on the other. The three sides are square-shaped and include moveable block letters, rotating gears and sliding shapes. The name “Imaginarium” appears in a blue, oval-shaped logo underneath the mirror on one end. The second ‘i’ in the logo is dotted with a shooting star. The recalled product has model number 46284; Toys “R” Us item number 295909; and has the barcode number 000799985462841. The model number is printed on the back of the box, directly above the bar code. These numerical markings do not appear on the actual product.

Sold exclusively at: Toys “R” Us stores nationwide and online via www.toysrus.com from August 2009 through September 2010 for about $25.

Remedy: Consumers should immediately stop using the product, move it out of the reach of young children and return it to a Toys “R” Us store for a refund or store credit.

Name of Product: Green Toys(tm) Mini Vehicles

Units: About 50,000 in the United States and 2,500 in Canada

Manufacturer: Green Toys Inc., of Mill Valley, Calif.

Hazard: The wheels and hubcaps on the toy cars can detach, posing a choking hazard to young children.

Incidents/Injuries: Green Toys has received 10 reports of wheels and hubcaps detaching or loosening from the vehicles. There have been no reports of injuries.

Description: This recall involves Green Toys(tm) Mini Vehicles. All cars are made of plastic and available in yellow, green, red, white and blue. Model names and numbers for the seven recalled vehicles or sets are listed in the table below. For Mini Vehicle (MVHA – 1014) and Fastback (MVFA-1022) sets the model numbers can be found on the bottom of the toys’ packaging. For all other cars, a label with the model number can be found on the bottom of the car itself.

Cars with an “I” etched into the underside of the car next to the date stamp are not part of the recall.

Sold by: Mass merchandisers, grocery stores and independent toy and specialty stores nationwide and by online retailers from April 2012 to June 2012. The cars cost about $5 each or $18 for the set.

Remedy: Consumers should take the product away from children immediately and contact Green Toys Inc. to receive a full refund.

Name of product: Contours Options LT Tandem Strollers

Units: About 5,600

Hazard: The front wheel assembly can break, posing a fall hazard to the child in the stroller. In addition, for strollers manufactured in January and February 2012, the nuts that hold the stroller’s basket support screws in place can detach. Detached nuts can pose a choking hazard to young children.

Incidents/Injuries: Kolcraft has received six reports of front caster wheels breaking and two reports of the basket’s support screws and nuts detaching. No injuries have been reported.

Description: This recall involves all Contours Options LT tandem strollers with model number ZT012. The model number and date of manufacture are printed on a label found on the rear leg of the stroller. The dual-seat strollers have one mesh basket beneath both seats and were sold in two color schemes; black with red canopies and accents, and gray with yellow canopies and accents. “Options LT” is printed on a bar along the side of the stroller.

Sold at: Burlington Coat Factory and juvenile product specialty stores nationwide and online at Amazon.com, Target.com and other online retailers between February 2012 and July 2012 for about $250.

Remedy: Consumers should immediately stop using the product and contact the company to receive free replacement wheels. Consumers with strollers manufactured in January and February 2012 will also receive replacement nuts for the basket support screws.

Description: The recalled rattles are made out of multi-colored, woven plastic and have a bell inside. They measure about 4 1/2 inches long. “Made in Mexico” and “Lee Carter Company” are printed on a tag on a purple plastic loop at the end of the rattle’s handle.

Sold at: Various Mexican specialty craft stores nationwide from February 2001 through October 2011 for about $4.
