JELD-WEN Recalls to Repair Interior Bifold Doors Due to Impact Hazard

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: JELD-WEN and Reliabilt Interior Bifold Doors

Units: About 170,800

Manufacturer: JELD-WEN, Inc., of Klamath Falls, Ore.

Hazard: The lower pivot pin can break causing the door to disengage from the overhead track, posing an impact hazard.

The lower pivot pin is a metal screw that fits into a white plastic sleeve that is inserted into the bottom edge of the door during the installation process. Only doors with pivot pins that have 12 rectangular indentations in the collar at the end of the sleeve are subject to the recall.

Incidents/Injuries: JELD-WEN is aware of twelve incidents of broken pivot pins causing the bifold doors to disengage from the overhead track, including three incidents involving minor injuries.

Description: The recalled molded white panel and flush natural wood grain bifold doors run on upper tracks and lower pivot pins. The lower pivot pin is a metal screw that fits into a white plastic sleeve that is inserted into the bottom edge of the door during the installation process. Only doors with pivot pins that have 12 rectangular indentations in the collar at the end of the sleeve are subject to the recall.

Sold at: Home Depot, Lowe’s and other building products retailers between February 2011 and March 2012 for about $40 to $80.

Remedy: Consumers should stop using the doors and contact JELD-WEN for assistance in identifying and obtaining free replacement hardware and technical assistance.

Atomic Aquatics Recalls Cobalt Dive Computer Due to Impact and Drowning Hazard

The unit can leak and cause the lens of the computer to blow off suddenly, which could result in impact injuries, and can cause a gas leak, posing a drowning hazard.

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Cobalt Dive Computer

Units: About 4,000 in the United States and 200 in Canada

Manufacturer: Atomic Aquatics, of Huntington Beach, Calif.

Hazard: The unit can leak and cause the lens of the computer to blow off suddenly, which could result in impact injuries, and can cause a gas leak, posing a drowning hazard.

Incidents/Injuries: The firm has received 23 reports of leaks caused by the lens being forcefully expelled from the computer due to excess air pressure inside the computer. No injuries were reported.

Description: This recall involves Atomic Aquatics brand Cobalt dive computers. These handheld, console-style dive computers have LCD screens and four magnetic control buttons located below the screen. A fitting at the bottom of the computer connects to the high pressure hose of a scuba diving regulator. The name Atomic Aquatics logo is imprinted on the front of the console, below the screen. The recalled products can be identified by the manufacture dates, which are determined by the first four numbers of the serial number with the first 2 digits signifying the week of manufacture (01 through 52) and the second 2 digits signifying the year of manufacture (10, 11 or 12). The dates of manufacture are between May 31, 2010 and April 16, 2012. Serial numbers, 2210-XXXX through 1612-XXXX, can be found by scrolling to the “System Info” screen on the computer.

The recall affects all Cobalt computers with serial numbers that start with any of the following numbers:

10 Series:   2210, 2910, 4010, 4710, 5010
11 Series:   1111, 1711, 2611, 2811, 3211, 3311, 3411, 3511, 3611, 3711, 3811, 3911, 4011, 4111, 4211, 4511, 4611, 5211
12 Series:   0812, 1112, 1212, 1312, 1412, 1612

Sold at: Authorized Atomic Aquatics dealers sold Cobalt dive computers from November 2010 through July 2012 for about $1200.

Remedy: Consumers should immediately stop using the recalled dive computers and return the unit to either an authorized Atomic Aquatics dealer or the Atomic Aquatics factory for inspection and repair.

Manufacturer: Crary Industries Inc. of West Fargo, N.D.

Hazard: The end cap of the log splitter’s hydraulic cylinder can break away from the body of the log splitter, posing an impact hazard to the user or bystander.

Incidents/Injuries: Crary Industries has received three reports of the hydraulic cylinder end caps detaching resulting in one injury from a cylinder reportedly striking a consumer in the head.

Description: This recall involves ECHO Bear Cat brand hydraulic log splitter with model numbers LS27270 and LS27270T, with a date code of 012908 stamped on the hydraulic cylinder. The model number is printed on the main beam of the log splitters. The date code is stamped on the hydraulic cylinder near the capped end. The log splitters are black and orange with ECHO Bear Cat printed on the hydraulic cylinder.

Sold at: ECHO Bear Cat dealers nationwide from October 2007 through March 2012 for about $2,600.

Remedy: Consumers should immediately stop using the product and contact the nearest dealer for instruction on receiving a free replacement cylinder installed by an authorized dealer.

Incidents/Injuries: Cooper Lighting has received 407 reports of trim pieces falling and breaking, including laceration injuries to a consumer’s head and foot when struck by the light’s trim and lens as they fell from the ceiling fixture.

Description: This recall involves All-Pro shower light trim models ERT701, RE-ERT 701, ERT702 and ERT703 with a glass lens and date codes 347 11 through 119 12. The model number and date code are marked on underside of the portion of the trim facing the ceiling. The trim is used with light fixtures listed below.

Flushmate has received 304 reports of the product bursting, resulting in property damage and 14 impact or laceration injuries.

Name of Product: Flushmate(r) III Pressure-Assist Flushing System

Units: About 2,330,600 in the U.S. and 9,400 in Canada

Manufacturer: Flushmate, of New Hudson, Mich., a division of Sloan Valve Company

Hazard: The system can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact or laceration hazards to consumers and property damage.

Incidents/Injuries: Flushmate has received 304 reports of the product bursting, resulting in property damage and 14 impact or laceration injuries.

Description: This recall is for Series 503 Flushmate(r) III Pressure Assist flushing systems installed inside toilet tanks. The recalled systems were manufactured from October 1997 to February 2008. The units are rectangular, black, two-piece vessels made of injection molded plastic. The date code/serial number is 16 characters long and is located on the label on the top of the Flushmate III. The first six numerals of the serial number are the date code. The date code range for this recall begins with 101497 (October 14, 1997) and continues through 022908 (February 29, 2008).

Sold at: The Home Depot and Lowe’s stores, distributors and plumbing contractors nationwide for about $108, and sold to toilet manufacturers including American Standard, Crane, Eljer, Gerber, Kohler, Mansfield and St. Thomas.

Remedy: Consumers should immediately turn off the water supply to the recalled Flushmate III unit and stop using the system. Consumers should contact the firm to determine if their Flushmate III serial number is included in the recall and to request a free repair kit.

Units: About 7,700 in the United States and 230 in Canada

Importer: Pottery Barn Kids, a division of Williams-Sonoma Inc., of San Francisco, Calif.

Hazard: The connections of the posts to the top rails of the canopy can come apart, allowing components of the canopy to fall and posing an impact hazard to consumers.

Incidents/Injuries: The firm is aware of 33 reports of components of the canopy falling, including one report of a cut that required stitches and nine other reports of minor cuts, bruises and/or bumps.

Description: The canopy is part of the Madeline Bedroom Collection. It is made of wood and consists of four posts that connect to the headboard and footboard of the bed and four rails that connect to the top portion of each post. It is available for twin-, full- and queen-sized Madeline Beds and comes in white, antique white, blade green, Sun Valley blade green, Sun Valley light green and Sun Valley espresso. Pottery Barn Kids previously sold the canopy separately as an option for the Madeline Bed.

Sold at: Pottery Barn Kids stores nationwide, online at potterybarnkids.com and through Pottery Barn Kids catalogs from December 2003 until August 2011. When sold separately, the canopy sold for $150 to $300.

Remedy: Consumers should immediately remove the canopy from the bed and contact Pottery Barn Kids to schedule a free repair.
