Description: The Perfect Bands are rubber tubes connected through a hole and secured by grommets to cloth sewn into loops.  The bands can be used alone or in conjunction with optional handles, which are sold separately.  The bands are approximately 40 inches long and are used for exercises described on the product packaging card and in a Workout Chart available on line on the product website.  ”Perfect Fitness” is printed on a tag on the cloth loop.  UPC codes can be found on the package. The recalled bands are:

They are used to attach a window to a frame and primarily sold for use in commercial buildings. The fittings have clamps, locking pins and stainless steel springs. “G-U” and the component number are stamped at the ends of the fitting. Incidents/Injuries: The firm has received four reports of the climbing sticks breaking. There have been no reported injuries.

Description: The recalled Muddy Outdoors climbing sticks are used to climb trees. They consist of a 20-inch black center stick with tree cleats and fold-out steps on each end. A black and burnt orange-colored rope fastened by a cam cleat secures the stick to the tree. The tree cleats have single metal tabs or posts. Model number 70401 or 70404 is printed on the packaging. The climbing sticks were sold individually or in packs of four.

Sold at: Bass Pro, Cabela’s, Dick’s Sporting Goods, Scheels and other hunting and outdoor stores nationwide from July 2012 through October 2012 for about $50 when sold individually and $160 when sold in a pack of four.

Hazard: The air rifles can fire unexpectedly when closing the action during the cocking process.

Incidents/Injuries: The firm has received one report of an air rifle that unexpectedly discharged while a consumer closed the action during the cocking process. No injuries have been reported.

Description: This recall involves Hatsan Striker air rifles that shoot pellets. The air rifles are available in three styles: black with a gray forearm grip and gray rear grip; camouflage with a black forearm grip and black rear grip; and, brown hardwood with crosshatching cut into the forearm grip and rear grip. They measure 43 inches in length. The air rifles have a black scope and a front sight and a rear sight. “Striker” is printed on the top of the gun’s barrel behind the rear sight. Serial numbers of recalled rifles begin with one of the following sets of numbers: 0511, 0811, 0911, 1011, 0112, 0212, 0312, or 0412. The serial numbers are printed on the air rifle’s barrel block on the left side just in front of the rear sight.

Sold at: Dunham’s Sports, Sportsman’s Warehouse and other sporting goods stores and gun shops nationwide and online and catalog sales including Airgun Depot, The Sportsman’s Guide and Pyramyd Air from November 2011 through July 2012 for between $100 and $130.

Reported Death, Severe Neck Injuries Prompt Recall of Banzai Inflatable Pool Slides Sold by Walmart and Toys R Us

WASHINGTON, D.C. – In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Wal-Mart Stores Inc., of Bentonville, Ark. and Toys R Us Inc., of Wayne, NJ are announcing the recall of about 21,000 inflatable Banzai in-ground pool water slides. During use, the slide can deflate, allowing the user to hit the ground underneath the slide and become injured. The slide is also unstable and can topple over in both still and windy conditions and carries inadequate warnings and instructions.

The CPSC is aware that a 29-year-old Colorado mother died in Andover, Mass. after fracturing her neck going down a Banzai in-ground pool water slide which had been placed over the concrete edge of a pool. The victim hit her head at the bottom of the slide because it had partially deflated.

The CPSC and the retailers are aware of two other injuries which have occurred in a similar manner, including a 24-year-old man from Springfield, Mo. who became a quadriplegic and a woman from Allentown, Pa. who fractured her neck.

The recall involves Banzai in-ground pool water slides designed for use with in-ground pools. The vinyl slides have a blue base, yellow sliding mat and an arch going over the top of the slide. By connecting a hose to the top of the slide, water can be sprayed on its downward slope. The words ‘Banzai Splash’ are printed in a circular blue, orange and white logo, shaped like a wave on either side of the slide.

The recalled slides, which were manufactured in China by Manley Toys, Ltd, were sold at Walmart and Toys R Us nationwide from January 2005 through June 2009 for about $250. The recalled slides have the barcode number 2675315734 and model number 15734. Both the barcode and model number appear on the original packaging but are not on the actual slide.

CPSC urges consumers to immediately stop using the product and return it to the nearest Walmart or Toys R Us for a full refund. Consumers can also cut the two safety warning notices out of the slide and just return that portion. Description: This recall involves GE Profile(tm) frontload washing machines with model numbers beginning with WPDH8800, WPDH8900 and WPDH8910. All serial numbers with these models are included in this recall. The washers were sold in gold, red and white colors. The model number is located on the washer’s right side near the bottom and behind the door near the door frame visible when the door is opened.

Description: This recall involves Haier 42-inch LED TVs with model number LE42B1380. “Haier” is printed on the front of the TV and the model number is printed on a label on the back of the unit.

Description: This recall involves circular, 7-inch diameter Portfolio aluminum reflector with glass lens designed to be inserted into a light fixture. This reflector is intended for use in indoor, commercial applications, such as office buildings, schools and shopping malls. There are four springs attached to the lip of the reflector. Reflectors marked with a barcoded stamp on their outside surface are not subject this recall. The recall includes reflectors with the following catalogue descriptions: 7080*G, 7081*G, 7280*G, 7281*G, 7380*G, 7381*G, 7780*G, 7781*G, MSP5429*, MSP5443*, OLD-7280KLI4G and 7-inch reflectors with glass lens with the “PDR” (Product Deviation Request or customized product designation) prefix; however, this description does not appear on the product. The recalled assembly is used in the following lighting fixtures: C7018*, C7042*, C7113*, C7118*, C7126*, C7142*, C7213*, C7218*, C7226*, C7242*HD7*, and MD7*.

Description: This recall involves Black & Decker(r) brand 12-cup coffeemakers designed to mount under a cabinet. Available in either white or black models, the words “BLACK & DECKER(r) SPACEMAKER(tm)” are printed in the top right-hand corner. The coffeemakers have a digital time display in the top left corner and five round buttons above the water reservoir. The 12-cup coffee pots are glass with a molded handle the same color as the machine and a silver metallic bracket running around the glass near the bottom.

The recall includes model numbers SDC740, SDC740B, SDC740BR, SDC740C, SDC740DIS, SDC740R, SDC750, SDC750C and SDC750DIS. The model number is printed on the underside of the coffeemaker, directly below the water reservoir.

Name of Product: Snowpulse Avalanche Airbags

Units: About 1,200 in the United States and 2,600 in Canada

Hazard: A leak in the airbag’s cartridge can result in the airbag not deploying, posing a risk of death and injury in the event of an avalanche.

Incidents/Injuries: None reported.

Description: This recall involves Snowpulse Avalanche airbags with inflation-system 1.0 air cartridges. The airbags are used for skiing, snowmobiling and mountain climbing to help keep the user above the surface if an avalanche occurs. Model year 2008 to 2010 airbag cartridges are included in this recall. The packs are between 15 and 45 liters and have the “Snowpulse” logo printed on them. The metal cartridge is inside the pack and unscrews from the airbag. Cartridges using inflation system 1.0 gauges can be identified by the pin inside the threaded fitting on the side of the cartridge. If this gauge does not have an “A” or a “B” on the dial then it is included in the recall.

Sold by: Specialty outdoor and motorsports stores nationwide from September 2008 through April 2012 for between $900 and $1,200.
