Five Retailers Agree to Stop Sale and Recall Tots in Mind Crib Tents Due to Strangulation and Entrapment Hazard
One Death and Serious Brain Injury Reported

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) and five retailers are announcing a voluntary recall to provide refunds to consumers who own crib tents and play yard tents made by Tots in Mind, Inc.

CPSC is warning parents and caregivers who own these products that infants and toddlers are at risk of serious injury or death due to strangulation and entrapment hazards presented by these products. Tots in Mind recalled the play yard tents in July 2010 and offered a repair kit that is no longer available. The company is no longer in business and has stopped all sales. CPSC staff urges parents and caregivers to stop using these crib tents and play yard tents immediately. Do not attempt to repair these products.

CPSC is aware of 27 tent failures including one fatality and one serious injury that occurred between January 1997 and April 2012 from crib tents and play yard tents made by Tots in Mind, Inc. In 2008, a two year old boy died after becoming entrapped between the bottom rail of a play yard tent and the top rail of a play yard. The fatality was reported in a prior recall with the firm in July 15, 2010.

In 2007, a two year old boy sustained a catastrophic brain injury when the crib tent affixed to his crib tent inverted and the product’s broken rod trapped him at the neck. The remaining 25 reports to CPSC involved inverted crib tents-entrapments between the tent and the crib/play yard or failures of the tent fabric and zippers. Three of these 25 reports also resulted in injuries; in one such case, a parent reported finding her child turning blue and entrapped between the product and the top rail of the play yard.


Since Tots in Mind, Inc. is out of business, retailers who sold these products have stepped up to offer refunds or store credit to consumers. The crib tents and play yard tents can present an entrapment and strangulation hazard to infants and toddlers if the dome portion inverts inside the crib or play yard, or if the product becomes partially detached from the crib or play yard. The recalled products were sold at numerous retail stores including Bed Bath & Beyond/Buy Buy Baby, Burlington Coat Factory, Toys R Us/Babies R Us, Walmart and online on websites including Amazon.com, for between $60 and $85.

Consumers should contact the store listed below where the crib tent was purchased to receive either a refund or store credit, depending on the retailer. If consumer is unsure of where the crib tent was purchased, see return policy for individual retailers below on their website:

The recall includes various models of about 330,000 crib or play yard tents. Consumers can identify their tent by the 2″x 1½” label with Tots In Mind logo located on the non-mesh portion near the top of the tent.

No model names or numbers are located on the tents, however they can be identified by pictures and the Tots in Mind logo on top of the tent.

Manufacturer: Meyer Corporation U.S., of Vallejo, Calif.

Hazard: The glass lid used with the 5-quart covered sauté pan can crack, break or shatter, posing a laceration hazard to consumers.

Incidents/Injuries: Meyer has received 65 reports of broken or shattered lids. No injuries have been reported.

Description: The defective lid is 11 inches in diameter with a rubber and stainless steel handle and a metal rim. The code “IMCP1108″ is stamped on the outside of the metal rim. The lid is part of the 13-Piece Circulon Premier Professional cookware set. The cookware is aluminum and stainless steel with rubber and stainless steel handles. In addition to the covered sauté pan, the sets include: 1-quart and 3-quart saucepans, a 4-quart saucepot, an 8-quart stockpot, 8.5-, 10- and 12-inch French skillets and four additional glass lids.

Sold Exclusively at: Costco retail stores and Costco.com from about October 2011 through March 2012 for about $200 for the complete set.

Remedy: Consumers should stop using the 11-inch glass lid and contact Circulon customer service to receive a replacement lid.

The blender consists of a six-inch tall, white electrical base with five, white speed-selector buttons labeled 1 through 5, one gray button labeled “Pulse/Off” and the logo “Kitchen Selectives(r)” in black letters on the front; a clear plastic pitcher with a handle with U.S. and metric volume measurement markings; a white plastic lid with a removable clear plastic lid stopper; and a white plastic blade assembly with two angled, stainless steel blades.

Sold at: Midwest retail stores Pamida and Sutherlands from June 2009 to April 2012 for about $20.

Description: The recycled silk floor mats measure 2 ft. by 3 ft. They were sold in a variety of color stripe combinations. “World Market” and “SKU 441536″ are printed on the mat’s white care label sewn into the bottom of the mat.

Sold exclusively at: Cost Plus World Market stores nationwide from June 2011 through May 2012 for about $17.

Incidents/Injuries: Kolcraft and CPSC have received five reports of injuries involving the hinge mechanism, including reports of three children whose fingertips were amputated and two adults whose fingers were either smashed or lacerated.

Description: This recall involves Kolcraft Contours Options three- and four-wheeled strollers. Strollers included in the recall have model numbers starting with ZL002, ZL005, ZL008, ZL015 and ZL018. On the ZL002 model, the model number and date of manufacture is printed on a sticker above the left wheel. On the ZL005, ZL008, ZL015 and ZL018 models, the model number and date of manufacture is printed on a label sewn into the edge of back of the stroller seat pad. The strollers were manufactured from January 2006 through November 2009 and sold in various color schemes.

Sold at: Juvenile product specialty stores nationwide and online at Amazon.com, Target.com and ToysRUs.com from January 2006 and June 2012 for between $150 and $160.

Gerber Recalls Machetes Due to Laceration Hazard

Name of Product: Gerber(r) Bear Grylls Parang Machetes

Units: About 119,000

Importer: Gerber Legendary Blades, of Portland, Ore.

Hazard: A weakness in the area where the handle meets the blade can cause the handle or the blade to break during use, posing a laceration hazard.

Incidents/Injuries: The firm received 24 reports of breakages, including one report of a laceration injury in Canada, which did not involve stitches.

Description: The recalled product is a curved blade machete with an overall length of 19.5 inches and a blade length of 13.5 inches. The handle is a dark gray textured rubber grip with wrist lanyard, orange trim and a stylized “BG” on it. The blade is marked with the “GERBER(r)” trademark and a stylized Bear Grylls trademark. The machete comes in a black nylon sheath with orange and gray trim. The machetes were sold separately or as one of the products in Gerber’s Apocalypse Survival Kit. The model numbers are on the package. Model numbers are: 31-000698, which has “Survival Series” printed on the package; and 31-001507, which was sold only at Walmart. Model number 30-0006010 is for the Apocalypse Survival Kit, which includes a Parang machete among other items in a foldable black cloth case with “GERBER” printed in orange on the inside right.

Sold at: Sporting goods stores nationwide and online from January 2011 through June 2012 for about $43 for the individual Parang machetes and $349 for the Apocalypse Survival Kits.

Description: This recall includes Animal Snap Bracelets in assorted metallic fabrics with animal figures, including: Seahorse, Frog, Snake, Starfish, Crab, Lizard, Dolphin and Fish. Manufacturing date codes between 07/11 and 01/12 are printed on a sewn-in label found on the inside of the bracelet. The label includes the Toysmith name and address. A round paper tag is attached to the product with the product name, Toysmith information, the words “Item # 2360 Animal Snap Bracelet,” SKU number and bar code.

Sold at: Cost Plus World Market, Cracker Barrel Old Country Store and other retailers from September 2011 to February 2012 for about $3.

Name of Product: Flushmate(r) III Pressure-Assist Flushing System

Units: About 2,330,600 in the U.S. and 9,400 in Canada

Manufacturer: Flushmate, of New Hudson, Mich., a division of Sloan Valve Company

Hazard: The system can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact or laceration hazards to consumers and property damage.

Incidents/Injuries: Flushmate has received 304 reports of the product bursting, resulting in property damage and 14 impact or laceration injuries.

Description: This recall is for Series 503 Flushmate(r) III Pressure Assist flushing systems installed inside toilet tanks. The recalled systems were manufactured from October 1997 to February 2008. The units are rectangular, black, two-piece vessels made of injection molded plastic. The date code/serial number is 16 characters long and is located on the label on the top of the Flushmate III. The first six numerals of the serial number are the date code. The date code range for this recall begins with 101497 (October 14, 1997) and continues through 022908 (February 29, 2008).

Sold at: The Home Depot and Lowe’s stores, distributors and plumbing contractors nationwide for about $108, and sold to toilet manufacturers including American Standard, Crane, Eljer, Gerber, Kohler, Mansfield and St. Thomas.

Remedy: Consumers should immediately turn off the water supply to the recalled Flushmate III unit and stop using the system. Consumers should contact the firm to determine if their Flushmate III serial number is included in the recall and to request a free repair kit.
