WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Name of Product: Signature Command(tm) (electronically) Controlled Direct and B-Vent Gas Fireplaces and Inserts

Units: About 15,000

Manufacturer: Monessen Hearth Systems Co., of Paris, Ky.

Hazard: A control component in the fireplaces and inserts can prevent the unit from lighting though gas continues to flow, posing a fire hazard.

Incidents/Injuries: None reported.

Description: This recall involves electronically controlled Majestic, Monessen and Vermont Castings direct-vent and B-vent gas fireplaces and inserts. Models included in the recall have serial numbers range from 11-X-027322 to 11-X-031501; 12-X-000275 to 12-X-015206, 11-P-032597 to 11-P-059389; and 12-P-000202 to 12-P-037583. The first two digits represent the year of manufacture, the letter represents the facility and the last six digits are sequential numbers, randomly selected. The serial number is printed on a rating plate, affixed to a cable inside the lower or side control door.

Sold at: Hearth dealers and distributors nationwide between October 2011 and October 2012 for between $1,030 and $8,160.

Remedy: Consumers should immediately stop using and turn off the fireplace using the master control switch on the command center which is located inside the control door. Consumers should contact Monessen Hearth Systems Company to arrange a free repair.

Haier America Recalls 42-inch LED-TVs Due to Risk of Injury

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Name of Product: Haier(r) 42-inch LED TVs

Units: About 5,000

Importer: Haier America Trading L.L.C., of New York, N.Y.

Hazard: The TV stand’s neck support can break and cause the TV to tip over, posing a risk of injury to the consumer.

Incidents/Injuries: Haier America and CPSC have received 184 reports of the TV stand’s neck cracking or breaking. No injuries have been reported.

Description: This recall involves Haier 42-inch LED TVs with model number LE42B1380. “Haier” is printed on the front of the TV and the model number is printed on a label on the back of the unit.

Sold at: Fry’s Electronics and other retail stores nationwide and online at Amazon.com and other online retailers from September 2011 through March 2012 for about $450.

Remedy: Consumers should immediately stop using and detach the TV from the stand and keep both the TV and the base in a safe location. Consumers should contact Haier America to request a free replacement stand neck to be used with the original base. Consumers with a wall-mounted TV may continue to use it but should request a free replacement stand neck in case the TV is used with the stand in the future.

Hazard: The electrical wires are reversed on the receptacles on the power strips, posing a risk of electrical shock.

The power strips come in a red package with “Wiremold-Series Part PX1001″ printed on the front. The manufactured dates of the recalled products are presented as product date codes January 2011 through June 2012, and are found imprinted on a circle on the bottom with an arrow pointing to the number of the month and the year “11″ or “12″ in the center. The model number PX1001, and UPC number 0 86698 00125 3, are imprinted on the bottom of the packaging. Power strips that have a star symbol printed in black ink on the packaging and on the back cover of the power strip are not included in this recall.

Avalanche Airbags Recalled by Snowpulse Due to Risk of Injury

Name of Product: Snowpulse Avalanche Airbags

Units: About 1,200 in the United States and 2,600 in Canada

Distributor: Mountain Sports Distribution, of Golden British Columbia, Canada

Hazard: A leak in the airbag’s cartridge can result in the airbag not deploying, posing a risk of death and injury in the event of an avalanche.

Incidents/Injuries: None reported.

Description: This recall involves Snowpulse Avalanche airbags with inflation-system 1.0 air cartridges. The airbags are used for skiing, snowmobiling and mountain climbing to help keep the user above the surface if an avalanche occurs. Model year 2008 to 2010 airbag cartridges are included in this recall. The packs are between 15 and 45 liters and have the “Snowpulse” logo printed on them. The metal cartridge is inside the pack and unscrews from the airbag. Cartridges using inflation system 1.0 gauges can be identified by the pin inside the threaded fitting on the side of the cartridge. If this gauge does not have an “A” or a “B” on the dial then it is included in the recall.

Sold by: Specialty outdoor and motorsports stores nationwide from September 2008 through April 2012 for between $900 and $1,200.

Remedy: Consumers should immediately stop using the recalled airbags and contact Snowpulse for a replacement cartridge.

Stage and Riser Caddies Recalled by Midwest Folding Products Due to Risk of Injury or Death
Child died when stage stored on caddy fell on her

A 3-year-old girl died in May 2011 in Greenville, S.C., from severe head trauma when a portion of a portable stage fell out of the caddy and onto her at a church.

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Midwest Folding Products, of Chicago, Ill., is announcing a voluntary recall of about 5,300 stage and riser caddies due to risk of injury and death to consumers. The caddy’s latches do not automatically close to secure the stages and risers when stored. Unsecured stages and risers can fall off the caddies and onto consumers.

A 3-year-old girl died in May 2011 in Greenville, S.C., from severe head trauma when a portion of a portable stage fell out of the caddy and onto her at a church.

This recall involves Midwest Folding Products’ caddies used to store collapsible stages and risers when they are not being used. The caddies are brown, gray or black metal and measure 44 inches long by 30 inches wide by 60 inches high. They hold six 96-inch long or eight 48 to 72-inch long stage or riser platforms.

Yellow caution and instruction labels are located on the caddy’s frame.

The recalled caddies were sold by Midwest Folding Products’ dealers to churches, schools, performing arts centers and various other organizations nationwide and online at www.midwestfolding.com from January 1989 through March 2012 for about $1,200. Midwest Folding Products is directly contacting known purchasers of the caddies.

Caddy owners and consumers should immediately stop using the recalled stage and riser caddies. Caddy owners should contact Midwest Folding Products to receive a free repair kit with new brackets that will allow latches to automatically return to the closed position on the caddies and new white warning and instruction labels.

Importer: Target Corporation, of Minneapolis, Minn.

Hazard: Lamps may overheat, causing the adhesive inside the lamp socket to melt and migrate into the bulb area of the socket. The cooled glue can adhere to the light bulb base and make the bulb difficult to remove which can result in a broken light bulb, posing a risk of laceration to consumers. Melted flammable glue that migrates onto the electrical components of the lamp poses a risk of fire.

Incidents/Injuries: Target has received six reports of glue on the lamp socket melting and migrating into the bulb area of the socket. No injuries have been reported.

Description: Four styles of the children’s task lamp are included in this recall with the names Striped, Sports, Dot or Flower Dot and have a label with the UPC number on the bottom. The lamps in this recall include:

Strangulations can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the shade, or when a child pulls the cord out and wraps it around his/her neck.

Wm. Wright has added its name to the manufacturers and retailers joining the voluntary recall announced in December 2009 of ALL Roman shades and roll-up blinds.

Description: This recall involves all Wrights® “my home make-it-yourself” Roman shade kits with item numbers 1451007001, 1451008001 or 1451008001A. The kit fits windows of a maximum size of 48″ W x 60″ L. It includes 15 yards of Roman shade tape, 15 yards of drapery cord, 4- inch “L” brackets, 18 screws, one cord cleat and instructions. The item number and “Wm. Wright Co.” can be found on the back of the package.

Sold at: Walmart, Jo-Ann Stores and other specialty textile and variety retail shops nationwide from December 2007 through June 2011 for between $20 and $25.

CPSC Alert: Consumers Urged To Stop Using phil&teds USA Clip-on Chairs Due To Serious Dangers Posed To Children

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) warns consumers that some “metoo” clip-on table top chairs, imported by phil&teds USA Inc., of Fort Collins, Colo. put young children at risk of serious injury due to multiple safety hazards. CPSC is urging consumers to stop using some metoo clip-on chairs immediately.

The product is an infant/toddler chair with a nylon fabric seat and a metal frame that clamps onto tables using two metal vise clamps. The upper part of each clamp rests on the table top and has either a rubber clamp pad on its underside or a rubber boot covering it. The chair is sold in three fabric colors – red, black and navy.

The clip-on chairs affected by this warning do not have plastic spacers between the table clamps and the front horizontal metal bar. The clip-on chairs that have plastic spacers between the table clamps and the front horizontal metal bar are under evaluation.

The company has refused to agree to a national recall of their hazardous product that is acceptable to CPSC. The company has offered a repair kit consisting of rubber boots to place on the upper clamp grips of the chairs. Consumers should be aware that CPSC has not approved a repair kit for this product, despite the firm’s prior statement that it was conducting a recall “in cooperation with the U.S. Consumer Product Safety Commission.”

CPSC is urging consumers to stop using the affected metoo chairs at this time in order to prevent the risk of injury to children.

The affected metoo chairs pose serious fall and amputation hazards to children placed in them. Children can suffer impact and head injuries when the chair detaches from the table and falls with them in it. CPSC staff is aware of numerous incidents involving the affected metoo chairs.

CPSC staff has determined that the clamps can detach from a variety of different table surfaces. Additionally, the chairs can detach when children move around or use their feet to push against other objects. Staff also determined that the lack of adequate space between the horizontal metal bar at the front of the chair and the clamps can cause children’s fingers to be severely pinched, lacerated, crushed or amputated if caught between the bar and the clamp when the chair detaches.

In addition to hazards with the affected clip-on chairs, the product packaging and instructions provide conflicting information. The product’s packaging and marketing information show the product being used in ways that may lead to the chair detaching from the table. However, the product’s instructions do not adequately warn against this type of use.

Tens of thousands of the affected metoo chairs may have been distributed since May 2006 for about $50 through philandteds.com, Amazon.com, Buy Buy Baby, Target, Toys R Us, other online retailers and a variety of independent juvenile specialty stores.
